Job Summary: You will assist clients to successfully continue, resume, or adapt their academic or vocational activities using the Individual Placement and Support (IPS) Model. Provides ongoing academic or vocational support after placement is secured. Provides assistance to clients with needed resources and navigating multiple social services and health systems. Links clients to appropriate care and ensures clients achieve their wellness and recovery goals. Assures departmental goals or objectives are accomplished within prescribed time frame by performing the job responsibilities personally.

What You Will Do:

  • Provide a comprehensive case management assessment that addresses client's life domains (i.e., housing, finances, transportation, legal services, vocational, employment, health care and family strengths/needs).
  • Link clients to appropriate care and resources in the community including resources that address specialized needs (i.e., agencies providing services related to HIV/AIDS, mental health disorders, chronic and acute health problems and problems stemming from involvement with the criminal justice system).
  • Assist clients in identifying and clarifying academic and/or vocational goals, ensuring competitive opportunities and wages.
  • Quickly identify and secure appropriate academic or vocational placement opportunities for clients based on individual goals, eligibility, and readiness.
  • Provide ongoing academic and/or vocational support after placement is secured.
  • Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collect information on consumers and reflect all demographic data.

What We Need to See:

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field form an accredited college or university.
  • Must understand FEP phase-specific and CSC care
  • Must have experience as a supported education or employment specialist.
  • Valid driver's license and must be insurable under the agency's automobile insurance policy.
