Job Summary:
As a Case Manager, you will provide case management services to eligible adults experiencing severe and persistent mental illness who have been accepted to the Bergen County Prosecutors Office Mental Health Diversion Program (MHDP). In this role, you will deliver individualized and goal-driven rehabilitative interventions in various life domains (financial, mental, physical and legal assistance) to increase stabilization and ensure adherence to legal requirements. You will also collaborate with the judges and the prosecutor's office in creating and delivering progress reports, participating in weekly team meetings, and attending court hearings on behalf of the client.


What You Will Do:

  • Serve as primary contact for MHDP participants, providing referrals, executing CAPs, tracking progress, and supporting program completion.
  • Conduct assessments at intake, during services, and at discharge; collaborate with providers on service planning.
  • Provide individual, group, and family counseling, maintain a caseload, and mentor new staff.
  • Develop and review service plans with measurable goals and timelines.
  • Monitor medication compliance and offer substance use, crisis, family, vocational, and insurance support services.
  • Use client feedback and a strengths-based approach and deliver services based on Risk of Hospitalization.
  • Complete all clinical documentation per standards and ensure compliance with billing and demographic requirements.

What We Are Looking For:

  • Bachelor's or Master's degree in Social Work, Psychology, or a related behavioral health field.
  • Must have one (1) year of post-bachelor's experience working with individuals with serious and persistent mental illness is required.
  • Must have more than one (1) year of relevant experience and familiarity with the legal or criminal justice system are preferred.
  • Valid driver's license and must be insurable by agency's automobile insurance for primary drivers.
