Location: Hackensack Homeless Shelter; 365 W Passaic St, Rochelle Park, NJ

On-site & community-based M-F 8am-4pm, 8:30am-4:30pm or 9am-5pm

Hourly/Salary: between ($46-49k)

Job Summary:
As a Case Manager for our Rapid Rehousing for Families and Individuals, you will provide housing search and procurement, and case management services to Bergen County families and individuals who are homeless as defined by U.S. Department of Housing and Urban Development (HUD). Households may be residing in temporary shelter placement at the Bergen County Housing, Health and Human Services Center (BCHHHSC), living in hotel/motels paid by an agency, or living in a place not meant for human habitation.


What You Will Do:

  • Collaborate with service providers to establish comprehensive assessments and treatment plans
  • Coordinate continuity of care within the agency, community, jails, hospitals, etc.
  • Formulate comprehensive treatment plans containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Engage clients in services and address their needs
  • Provide linkages to health care services, dental care, benefits, mental health services, substance abuse services, family consultation, crisis intervention and vocational/educational/employment services; etc.
  • Communicate client progress, needs and concerns through case review, collaboration meetings and documentation
  • Assist clients in completing appropriate forms to ensure their basic financial needs are met (i.e. Medicaid, SSI, SSD, food stamps, GA, etc.)


What We Are Looking For:

  • Must possess from an accredited college or university a Bachelor's degree with 1-year experience working with individuals who are homeless or at-risk of homelessness
  • Experience in one or more of the following: crisis intervention, counseling, mental health, substance abuse, domestic violence, housing procurement, or job development
  • Valid driver's license and must be insurable by agency's automobile insurance for primary drivers.
