Job Summary:
As a Case Manager for our Housing Supportive Services program, you will provide case management services to NJ Family Care members at risk of homelessness suffering severe mental illness and/or chronic medical conditions with housing, tenancy maintenance, and sustainability. In this role, you will assist individuals in screenings, intake assessments, eligibility requirements and paperwork, and provide outreach services to individuals in the community to maintain housing stabilization on a varying low-high level basis dependent on client needs. You will also assist in crisis intervention, client education, supplying resources, referrals, report writing, and record-keeping.


What You Will Do:

  • Act as a "first responder" addressing complex crisis involving both psychological and environmental components
  • Work holistically, ensuring that care and discharge plans meet the physical, social and emotional needs of the client
  • Create care plan with client and coordinate all the client resources, as needed
  • Assist individuals with housing assistance
  • Complete a comprehensive assessment of the client's needs and identifies resources, including basic needs and assists in accessing these services
  • Document all services in the clinical record, with all entries authenticated and time/dated
  • Link clients to community resources, as needed
  • Maintain client confidentiality and ethical boundaries as per state regulations and agency policies
  • Provide support for team colleagues, and engage in program-related performance improvement processes and activities
  • Engage in inter-organizational care coordination and follows up on referrals when needed and/or indicated
  • Attend regularly scheduled multidisciplinary team meetings (MDT), as assigned
  • Prepare and submit documentation required for billing authorization as needed

What We Are Looking For:

  • Bachelor's degree in health, psychology, counseling, social work, education or other behavioral health profession from and accredited college or university
  • Must have experience and skill in competently performing case management activities
  • Must have at least one (1) to two (2) years' experience working with the homeless or at-risk population and/or with high need individuals with SMI/COD/SUD
  • Valid driver's license and must be insurable by agency's automobile insurance for primary drivers.
