Case Manager MWRAP

Job Summary:

You will be responsible and accountable for providing intensive and direct case management services for assigned client caseload. You will work with women and their families to support and strengthen their capacity to participate in health practices and maintain stable homes through a family-centered approach.


What You Will Do:

  • Provide client with a comprehensive case management assessment that includes life domains, such as housing, finances, transportation, legal services, vocational, employment, health and behavioral health care, and family strengths and needs
  • Promote information sharing between all providers on your client's progress and challenges with the informed consent of each client in prenatal and nutritional care areas.
  • Link client to appropriate care and resources in the community, including resources that address specialized needs
  • Coordinate needed interventions and services provided by multiple agencies
  • Remain in regular contact with the client and conduct home visits
  • Refer and increase connections to supportive services, and work with Perinatal Cooperatives/Risk Reduction Specialists in educating the medical community partners on the stigma around pregnant women with a SUD
  • Prepare client for birthing, including potential for child welfare involvement, NAS if on MAT or using opioids, accessing pediatrician familiar with prenatal exposure, preparing for living and care arrangements for infant
  • Provide support and link families with appropriate services such as early intervention, child services, and services accessible through the NJ Prevention System of Care
  • Organize team meetings that bring all services together on behalf of the client from pregnancy through postpartum and maintain contact with service providers that include all important information and updates


What We Need to See:

  • Bachelor's Degree from an accredited college or university in health, psychology, counseling, social work, education, or other behavioral health professions.
  • At least three (3) years of experience working with high-need families with substance use and mental health disorders. They know about systems that provide services to women and their families, such as maternal health, early childhood intervention, and child welfare.
  • Training in trauma-informed services.
