Job Summary:
You will provide supportive and clinical services to young adults while addressing basic housing needs and co-create and implement a treatment plan addressing the youth's basic and clinical needs, goals, and progress-planned services. Additionally, by performing the job responsibilities personally, you will maintain accurate and timely records for each case and ensure departmental goals or objectives are accomplished within the prescribed time frame.
What You Will Do:
- Provide supportive and clinical services to young adults while addressing basic housing needs
- Complete comprehensive assessments/evaluations of the client at intake/admission, during ongoing treatment/services delivery, and at discharge/transfer
- Formulate psychosocial treatment plans with relevant goals, behaviorally-measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
- Coordinate and follow through with clinical care recommendations made by the program clinician.
- Meet with youth six days per week to ensure that concrete needs are met and to provide in-home services and supportive interventions, including case management, linkage to community resources, provide inter-agency coordination of treatment, and respond to clients for crisis intervention.
- Develop and implement a life skills education program
- Serve as an advocate for the rights and needs of the youth
- Undertake any activities that facilitate the client's sustainability in the community
- Provide leadership within the team, engages in inter-organizational care coordination, and documents service outcomes
- Participates in quality assurance activities
What We Need to See:
- Bachelor's degree in a behavioral health discipline from an accredited college or university and documentation of participation in a master's program
- One (1) year of experience working with children and adolescent populations
- Valid driver's license and must be insurable by the agency's automobile insurance
