Clinical Care Coordinator

Job Summary:
You will provide comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning. Additionally, you will provide all services consistent with SAMHSA's fundamental components of recovery, in a person- and family-centered approach that incorporates cultural appropriateness.


What You Will Do:

  • Engage consumers and their families.
  • Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness (physical and mental), health promotion, prevention, skill-building and related health and wellness/recovery topics.
  • Offer on-site as well as outreach services (including transport of consumers).
  • Fulfill case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth.
  • Provide services based on the tenants of evidence-based practices (i.e., IMR, MI and TIC).
  • Ensure consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role.
  • Communicate observations and information about the consumer in the consumer's service record and to team members.
  • Engage in supportive counseling and crisis intervention.
  • Participate in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge.
  • Provide input into formulation of consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives.
  • Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers and reflects all demographic data.


What We Need to See:

  • Bachelor's Degree in Social Work, Psychology, Counseling or related field
  • At least 1 year of experience in a related field.
  • A valid driver's license and must be insurable by agency's automobile insurance
