Community Support Services Clinician (Per Diem)

Job Summary:

You will be responsible and accountable for providing a variety of mental health treatment services including assessment, diagnosis, treatment planning, and therapeutic services.


What You Will Do:

  • Conduct in-community or in-office assessments to individuals referred to or enrolled in Community Support Services by face-to-face clinical interview and review of information from collateral sources.
  • Provide and/or complete new consumer forms as required during initial assessment.
  • Provide comprehensive assessment reports to support medical necessity of services and guide the development of the Individualized Rehabilitation Plan.
  • Provide direct service to consumers to formulate the Individualized Rehabilitation Plan (IRP), which supports the level of need assessed and authorizes services for billing. Consult and collaborate with direct care staff as it relates to consumer needs and interventions to be rendered per the IRP.
  • Assess, diagnose, treat and counsel clients, develop individualized treatment plans that define services and interventions, and update treatment plans according to documentation requirements.
  • Ensure that all documentation is completed in electronic health record with accuracy, timeliness, and in accordance with Medicaid and New Jersey Division of Mental Health Services regulations.


What We Need to See:

  • Master's Degree in Social Work, Counseling or related discipline from an accredited educational institution.
  • Licensure: Licensed as an LAC, LPC, or LCSW in the State of New Jersey.
  • Documented knowledge of at least one (1) year work experience (Clinician) working with adults diagnosed with mental illness.
  • Have a valid driver's license and must be insurable under Agency's automobile insurance policy
