Job Summary:
You will provide outpatient therapeutic sessions to adults, children, and families to increase stabilization, maintain accurate and timely records for each case, and ensure departmental goals or objectives are accomplished within the prescribed time frame by performing the job responsibilities personally.
What You Will Do:
- Document service outcomes as per Care Plus and payer's policies and procedures
- Assess and evaluate clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer
- Provide services and supportive interventions, including case management, link to community resources, and provide inter-agency coordination of treatment.
- Respond to clients for crisis intervention
- Plan for communicating and documenting discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum.
- Assist client with concrete services and advocacy, interface with other systems on behalf of the client, provide and engage in inter-organizational care coordination, and follow up on referrals when indicated by initial and ongoing assessment
- Assist with problem-solving for individual and family member treatment and supportive interventions.
- Supervises students or junior staff as assigned by supervisor and document supervision as required.
- Provides educational/supportive group presentations and discussions with clients and/or family members focused on mental illness, chemical dependency/addictions, and/or dual diagnosis.
What We Need to See:
- Master's degree in social work or other behavioral health disciplines from an accredited college or university
- 2 years of clinical experience and documented knowledge of a least one (1) year of work experience counseling substance use and/or co-occurring populations
- MCO panel eligible, as applicable
- Prof. License as required by the program's requirements (LAC, LSW, and LCADC)
- A valid driver's license and must be insurable by the agency's automobile insurance.
