Families First Counselor - Per Diem

Job Summary:

  • Provides in-home services and supportive interventions including, case management, linkage to community resources, and inter-agency coordination of treatment.
  • Conduct assessments and evaluations of clients at intake, during ongoing service, delivery, and termination
  • Formulate relevant goals with behaviorally measurable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement.
  • Prepare written documentation of assessments, interception plans, progress notes, and discharge summaries in the clients' files with all entries authenticated with dates and times.
  • Plan for, communicate, and document aftercare plans for clients completing program and/or accessing other services in the continuum.
  • Collaborate as a member of an interdisciplinary team, including but not limited to, active participation in program and team meetings, providing support to team colleagues, and engaging in program-related performance improvement processes and activities.
  • Participate in weekly team case conferences and trainings as per Family Preservation Services' requirements.
  • Provide 24/7 coverage to caseload.

What We Need to See:

  • A Bachelor's degree or a master's degree in social work or other behavioral health discipline from an accredited college or university
  • Two (2) years of experience providing counseling to children and families.
  • A valid driver's license must be insurable by the agency's automobile insurance.
