Job Summary:
As a Care Coordinator in our ARRIVE Together Department, you will work within the Bergen County Psychiatric Emergency Screening Program where you will assist with answering our 24/7 crisis hotline. You will provide follow up outreach and/or on-site support services, care coordination, and psychoeducation to individuals who have been transported to the Screening Center who may benefit from follow up support services in the community upon discharge. You will assist with Law Enforcement in three local municipalities will also identify high users of their municipality's emergency services however, do not require hospitalization or transport to an emergency room, but may benefit from linkage to additional resources or services in the community.


What You Will Do:

  • Follow up with individuals at risk via telephone, in-person, or virtually
  • Provide supportive counseling, referral and/or linkage for individuals and support systems
  • Develop working relationships with Bergen County Emergency First Responders
  • Provide psycho-education as appropriate
  • Facilitate linkage to community resources/services as appropriate
  • Work collaboratively with the Psychiatric Emergency Screening Program team


What We
Are Looking For: 

  • Must have a minimum of a BA/BS degree in mental health, and be interested in working with adults with mental illness, experience preferred
  • 1 year of post-bachelor's experience working in a mental health related field
  • Strong motivation to learn and work independently to develop the role to meet the needs of the Bergen County community
  • Work well independently and within multidisciplinary teams, as well as collaboratively with law enforcement and emergency service personnel
  • Valid NJ Driver's license and eligible to be insured under CPNJ automobile insurance policy
