On-site Part-time 20 hours: Program runs Between hours of M-F 8:00 am - 4:00 pm.
Hourly: $20.50

Job Summary:
As a Case Manager in our Behavioral Health Home, you will have the opportunity to work with adults in Bergen County with serious mental illness and chronic medical conditions, with the goal of improving their physical wellbeing through integrated care coordination through the use of various evidence-based modalities (such as CBT, MI, etc.).
What You Will Do:
- Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness
- Offer on-site as well as outreach services (including transport of consumers) and fulfill case management services
- Ensure consumers have community support in place that fulfill a positive corroborative role
- Communicate observations and information about the consumer in the consumer's service record and to team members
- Participates in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
- Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
- Implement evidence-based interventions (such as CBT, MI, etc.) and assessments to help individuals achieve their treatment goals, link to community resources, and provide inter-agency coordination of treatment.
- Participate in quality assurance activities and cross-functional action teams
What We Need to See:
- Bachelor's degree in a related field
- Two (2) years of experience working within a health care or behavioral health setting.
- A valid driver's license and must be insurable by the agency's automobile insurance
