Job Summary:
Provide supportive family and/or individual contacts, case management, assessments, intervention planning, client and/or family education, and aftercare planning. Assures program goals and objectives are accomplished by fulfilling job and team responsibilities as assigned.

What You Will Do:

• Assess and/or evaluate clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge/transfer
• Respond to clients for crisis intervention
• Function within an interdisciplinary team in the assessment and/or evaluation of clients, and to articulate verbally and in written documentation the integration of client history and assessment information.
• Provide leadership within the team, document services in the clinical record, engage in inter-organizational care coordination, and document service outcomes
• Formulate psychosocial treatment plans, with relevant goals, behaviorally-measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
• Plan, communicate, and document discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
• Document assessments, treatment plans, progress reports, and discharge summaries in the clinical record, with all entries authenticated and time/dated

What Makes You an Ideal Candidate:

• Master's degree in a behavioral health discipline from an accredited college or university
• 1-2 years of clinical experience in the mental health field, including but not limited to internships, community-based services, case management, etc
• Must have the ability to respond to clients for crisis intervention.
• Provisional/Professional Licensure to practice counseling/clinical social work (LAC, LSW, LPC, LCSW, LMFT)
• A valid driver's license and you must be insurable by the agency's automobile insurance
