Job Summary:

As a case manager and member of the Kinship Team, you will help provide families and caregivers with linkages to government and community resources to reduce the risk of children's placement with the Division of Child Protection and Permanency. Work with the families and caregivers to identify gaps, educate caregivers, and act as a liaison with vendors, stakeholders, and community partners with the end goal of maintaining residential permanency for the children.


What You Will Do:

  • At time of first initial request for services and on an on-going basis, evaluate and assess current and immediate needs of kinship families relating to family's goals and needs
  • Conduct home visits, collect documentation, and complete written assessment
  • Establish a trusting relationship with families
  • Engage and provide support to families through the information and referral process
  • Guide caregivers through the application process, including conducting an initial hierarchical needs assessment as well as providing additional information for referral to appropriate services
  • Act as a liaison between caregivers and the program's contractor, the appropriate services providers
  • Promote advocacy skills, empowering clients through appropriate program and service education, including how to effectively communicate with potential providers to ensure client's individualized needs are met
  • Complete documentation within the program's allotted timeframe, as set by Program Manager and Program deliverables

What We Need to See:

  • Bachelor degree in social work or other behavioral health discipline from an accredited college or university
  • Documented knowledge of one (1) year work experience with children and adolescent populations
  • A valid driver's license insurable by the agency's automobile insurance.
