Job Summary:
You will provide case management services, including, but not limited to, screening, intake, orientation, assessment, crisis intervention, client education, referral, report and recordkeeping.
What You Will Do:
- Act as a "first responder" addressing complex crisis involving both psychological and environmental components
- Work holistically, ensuring that care and discharge plans meet the physical, social and emotional needs of the client
- Coordinate all of the client resources, as needed
- Assist individuals with housing assistance
- Complete a comprehensive assessment of the client's needs and identifies resources, including basic needs and assists in accessing these services
- Document all services in the clinical record, with all entries authenticated and time/dated
- Create a care plan with client
- Link clients to community resources, as needed
- Maintain ethical boundaries with clients and staff
- Maintain client confidentiality as per state regulations and agency policies
- Provide support for team colleagues, and engage in program-related performance improvement processes and activities
- Engage in inter-organizational care coordination and follows up on referrals when needed and/or indicated
- Attend regularly scheduled multidisciplinary team meetings (MDT), as assigned
- Prepare and submit documentation required for billing authorization as needed
What We Need to See:
- Bachelor's degree in health, psychology, counseling, social work, education or other behavioral health profession from and accredited college or university
- Must have experience and skill in competently performing case management activities
- Must have at least one (1) to two (2) years' experience working with the homeless or at-risk population and/or with high need individuals with SMI/COD/SUD
- Valid driver's license and must be insurable by agency's automobile insurance for primary drivers.
